Isolation or low mood happens. Years of both: linked to thinking problems.

People say: “Do years of isolation and low mood leave a mark later?” We read the study.

What they did

Cohort study. 1,196 people. court records 1967-1971 (ages 0-11), interviews 1989-1995 (mean age 29) to 2022-2023 (mean age 59).

People were watched for years. Nobody was told what to do. A clue, not proof.

Who: Children from a US Midwestern metropolitan area with court-documented physical abuse, sexual abuse or neglect before age 12 (1967-1971) plus controls matched on age, sex, race/ethnicity and approximate family social class; 1,196 interviewed at mean age 29 (62% White non-Hispanic, 33% Black non-Hispanic), 447 assessed at mean age 59. Predominantly low socioeconomic status

What they found
  • Isolated and depressed+41.5%for years; linked
  • Only one, moderatelyno clearnot clearly higher
  • Abused or neglected+21.2%as kids, on record
The catch

More to know, from our read of the paper:

  • Observational: linked, not proven. Group 4 aRR 1.415 (95% CI 1.066-1.879), adjusted for age, sex, race/ethnicity, education, childhood maltreatment, hypertension, diabetes, obesity, limited physical activity, hearing impairment, smoking, heavy drinking and traumatic brain injury; APOE genotype not available
  • Social isolation is an objective-ish count of social activities never done + not married/cohabiting (7 items, alpha .66), NOT loneliness. Say 'isolated', never 'lonely'. Depression = symptom scales (DIS-III-R at 29, self-report CES-D later), standardised to z-scores
  • CIND here is a broad research classification: any one of 12 tests >= 1.5 SD below norms without functional dependence. 57.3% of the cohort met it, so it is not a diagnosis of MCI; say 'thinking problems short of dementia' / 'thinking test problems', not 'dementia'
  • Only 447 of 1,196 had the age-59 assessment; missing outcomes were multiply imputed (MAR). Delta-adjusted MNAR sensitivity: Group 4 association attenuated but stayed significant
  • Group 4 ('chronically isolated and depressed') is small: n = 49 (4.1%). Groups 2 ('moderately isolated') and 3 ('moderately depressed') did not differ significantly from Group 1 in the trajectory model
The verdict

Years of both together: linked to higher risk.

In short

This US cohort followed 1,196 people from a Midwest city, about half with court records of abuse or neglect before age 12 (1967 to 1971) and half matched children without such records. Between ages 29 and 47 they reported depression symptoms and how isolated they were; around age 59, 447 took a battery of thinking tests (missing results were estimated statistically). The 4% who stayed both isolated and depressed had a 41.5% higher risk of thinking problems short of dementia than those who were neither, after adjusting for risks like blood pressure, diabetes, smoking and education. Being only moderately isolated or only moderately depressed was not clearly linked, and documented childhood abuse or neglect was linked to 21.2% higher risk. Linked, not proven.
